Ronda Wenger speaks with staff and residents in nursing homes to identify people who wish to transition back to the community. Ronda gives presentations to community organizations, senior festivals, church groups, and medical teams explaining how the Nursing Facility Transition Program can assist in transitioning people out of facilities. She has a Masters Degree in Social Work. She has worked for the Single Point of Entry as an Options Counselor, in two nursing homes as discharge planner, and as an outreach specialist for the NFT program. Many of the people Ronda helps transition out of nursing homes are between the ages of thirty and fifty. Some are able to return to work or find employment in different fields following physical, occupational, and speech therapy in rehabilitation units of the facilities.
