Nursing Facility Transition assists with an individual’s transition back to the community through:

• Meeting to develop a plan with goals and objectives
  
• Assisting with housing location/modification
  
• Providing some financial assistance for essentials
  
• Identifying community resources
  
• Six months follow up after the transition

Nursing Facility Transition is for those who would like to return to their own home, move in with a family member, find an apartment, or choose another option.

Nursing Facility Transition is available for residents of nursing facilities who are Medicaid eligible.

Financial assistance is available for some one-time moving expenses such as a security deposit, housekeeping supplies, and essential furniture.
